Common Types of Never Events
The National Quality Forum (NQ F) recognizes 28 never events that are broken up into seven categories: surgical events, medical device events, issues with patient protection, care management issues, events caused by environmental factors, radiological events and criminal harm to patients.
Listed below are three never events out of the total 28 events:
- Patient Suicide: If a patient commits suicide or attempts to commit suicide while under the care of a medical facility, this is considered a never event. Patients should be closely monitored at all times, leaving no room for suicide/attempted suicide.
- Sexual Assault on a Patient: This is a type of criminal never event that results from the oversight of a medical facility. When a patient is not monitored carefully, a medical facility could be held responsible for any sexual assault on that patient.
- Surgery Performed on the Wrong Patient: This is a type of surgical never event. A surgeon may perform the correct surgery, but on the wrong patient.
